Eligibility and Services
Who qualifies for free ECM services?
You must be an active Medi-Cal member enrolled in Molina, LA Care, or Health Net, and live in Los Angeles, Riverside, or San Bernardino County. Additional qualifying factors include long-term health conditions, recent hospital stays, or limited social support.
What if I don't qualify for free services?
We offer self-pay options for people who don't meet the free service eligibility requirements. Please call (626) 669-6777 to discuss pricing and available services.
Do you provide services in my home?
No, all our services are provided outside the home. We give you rides to medical facilities, government offices, and community resources, providing support, translation, and coordination services at those locations, then safely return you home.
Which managed care plans do you work with?
We work with Molina Healthcare, LA Care Health Plan, and Health Net. You must be enrolled in one of these plans to qualify for free services.
Transportation and Appointments
How does transportation coordination work?
We provide door-to-door transportation for medical appointments, pharmacy visits, and important services. Our team picks you up from your home, coordinates pick-up times, makes sure you arrive safely and on time, and safely returns you home after your appointment.
Can you help with translation during medical visits?
Yes, we provide real-time translation and interpretation services during appointments at medical facilities, making sure you can talk clearly with doctors, nurses, medical staff, and office workers. We go with you into appointments and provide language help on-site.
How far in advance should I schedule services?
We recommend scheduling routine appointments at least 48-72 hours in advance. For urgent medical needs, we can typically accommodate same-day requests.
Will you go with me into my medical appointments?
Yes, our community health workers give you rides to medical facilities and can go with you into appointments, providing translation services, helping with paperwork, and making sure you understand medical information and instructions while at the medical facility.
Benefits and Housing Help
What types of benefits do you help with?
We help with SSI (Supplemental Security Income), EBT (Electronic Benefit Transfer), housing applications, Medi-Cal renewals, and other government assistance programs. We give you rides to government offices and go with you to complete application processes.
Can you help me find senior housing?
Yes, we provide practical help for finding, applying for, and getting affordable senior housing options within our service areas. We give you rides to housing offices and property visits, going with you through the application process.
How long do benefit application processes take?
Processing times vary by program. SSI applications can take 3-6 months, while EBT and housing applications may process faster. We keep you updated throughout the process.
Family and Caregiver Support
Do you help family caregivers?
Yes, we provide help and resources for adult children or relatives managing family member healthcare needs. This includes medical appointment coordination, transportation arrangements, and connecting families with other resources. We can give your family member rides to appointments and provide on-site support.
Can multiple family members receive services?
Each person must meet eligibility requirements individually. However, we can coordinate services for multiple eligible family members.

How much do your services cost?
Services are 100% free for eligible Medi-Cal members. For those who don't qualify for free services, we offer self-pay options.
